In reply to Razel. You can have a look at the new website of the PCG Consulate. This is the only information that I found regarding maiden surname: "If an applicant’s marriage has been dissolved and wants to revert to her maiden surname . . ." Unfortunately, that's all I found so far; I hope things will go well.

Hi Carolyn, I am planning to book an appointment for renewal of my passport in PCG Calgary. Unfortunately, I have an issue on how to secure an “Affidavit that applicant has opted to use maiden name” as listed in the consulate website as one of the requirement for married female applicant who won’t be using there husband’s surname on the passport. It’s a big help if you can help and advice me how to obtain the said document prior to my scheduled appointment since I have to travel far. I’m currently living in Grande Prairie. Thank you.

In reply to Catherin Marie Nudalo. Catherine you may call or email the Consulate as soon as possible. I will also pick up my passport in the Consulate on the date written on the recipe. I think it will take between 2 to 3 months to get the new passport. Ask when can you pick up yours before going to the Consulate since we cannot check online anymore if the passport is ready for pick up.

Hi, Carolyn! I’d like to know if it’s still possible if I’ll just pick up my passport in the Calgary Consulate instead of having it mailed? I live in Edmonton and went to Calgary two weeks ago to have my passport renewed. And since we suddenly moved, I no longer have an access to our old address. If it is possible, do you have any suggestions of what’s the best thing that I should do? Also, how long is the passport renewal process? Thank you!

In reply to Joan. The extension of passport validity was discontinued. You could renew your passport in the Philippines since it is advisable to travel with a passport having 6-month validity to avoid any inconvenience at the airports.

Hi! Do you have any idea if I can request at the consulate to make an extension with the validity of my passport? I am a permanent resident of Canada and my Philippine passport will expire February 2019 and I am traveling to the Philippines on December 7, 2018 and be back to Canada January 1, 2019. If so, do I have to make an appointment? Thanks
