Unfortunately, I have an issue on how to secure an &quot;Affidavit that applicant has opted to use maiden name&quot; as listed in the consulate website as one of the requirement for married female applicant who won&#039;t be using there husband&#039;s surname on the passport. It&#039;s a big help if you can help and advice me how to obtain the said document prior to my scheduled appointment since I have to travel far. I&#039;m currently living in Grande Prairie. Catherine you may call or email the Consulate as soon as possible. I will also pick up my passport in the Consulate on the date written on the recipe. I think it will take between 2 to 3 months to get the new passport. Ask when can you pick up yours before going to the Consulate since we cannot check online anymore if the passport is ready for pick up.]]></description>

I live in Edmonton and went to Calgary two weeks ago to have my passport renewed. And since we suddenly moved, I no longer have an access to our old address. The extension of passport validity was discontinued. You could renew your passport in the Philippines since it is advisable to travel with a passport having 6-month validity to avoid any inconvenience at the airports.]]></description>
